H1N1 vaccines available for Carey students

The South Central Public Health District (SCPHD) will be holding a free H1N1 vaccination clinic this Thursday, Nov. 12 from 1 - 4 p.m. at Carey School.

Parents who responded to a request sent home earlier this fall indicating they wanted their child vaccinated will receive first priority at this clinic. SCPHD has received enough vaccines to accommodate those who have signed up, including siblings ages 3 ‐ 18.

A parent or guardian is required to be present when their child is vaccinated. If children attend more than one school, all of them may receive the vaccination at this clinic. Everyone seeking a vaccination at the clinic will need a completed and signed consent form. While the forms will be available at the clinic location, they can also be downloaded from the SCPHD website, www.phd5.idaho.gov by clicking on the "H1N1 info for parents" button.

This is a closed clinic for pre-K through12 students of Carey School and their siblings. Contact Blaine County School District Director of Safety Scott Manning at 578‐5447 or the Carey School office for more information.
